PT Induced and maintained on isoflurane. Wing wrap was removed along with tegaderm patch. Surgery site looked good. No signs of dehiscence or discharge. The wing was placed through passive range of motion and massaged for approximately 5 min. The fracture felt stable. Cast padding was placed over the fracture site and the wing was wrapped in a figure 8 pattern with vetwrap. Recovered from anesthesia smoothly and encountered no complications.

Induced and maintained on isoflurane and heart rate and respiratory rate were monitored for the duration of the procedure. No anesthetic complications were encountered.
Removed bandage. There was exposed bone at the site of the wound. The exposed portion was debrided with bone rongeurs. Two simple interrupted sutures of 5-0 PDS were placed to close the wound. Minimal blood loss during the procedure. PROM was performed The wound was then covered by a telfa pad, tegaderm, and foam tape. A Sam splint was affixed with silk tape and the wing was wrapped in vetrap in a figure 8 pattern. CG/AMS

The patient was positioned in dorsal recumbency. The periocular
feathers around the right eye were plucked, and the right eye
and surrounding skin were then prepped in standard fashion with
dilute Betadine and eye wash solution. Proparacaine was not available.
A hemostat was placed to crush the tissue at the lateral canthus,
and then Stevens tenotomy scissors were used to create a lateral
canthotomy. A stab incision was made in the perilimbal cornea
with a 65-Beaver blade, and the entire cornea was then removed
by sharply dissecting it from the limbus using curved Wescott
tenotomy scissors. The uveal and retinal tissue was then bluntly
dissected from the sclera using a cyclodialysis spatula, and
the lens and vitreous were removed with cotton-tipped applicators.
The scleral shell was then copiously flushed twice with dilute
Betadine. No remaining intraocular structures were observed,
and the scleral wall was found to be unremarkable. Some of the
ventral palpebral conjunctiva was removed with Wescott tenotomy
scissors, but no attempt was made to ensure complete removal
of the conjunctival tissues. The eyelid margins were excised
with Stevens tenotomy scissors. Closure was performed in two
layers using 5-0 Vicryl in a simple continuous pattern in the
SQ and an intradermal pattern in the skin. No complications occurred
intraoperatively.

------------- LASER THERAPY ---------------- Power (joules): 504 Time: 5:36 Location: Right Metacarpal Region Notes: PROM was performed while the patient was conscious at aproximately 11:10am. The elbow's range of motion was 150 degrees post-PROM. The carpus' ROM was 180 degrees. The right metacarpal region was examined and the surgery site looked clean and without infection, sutures were holding up well, and there was a firm fibrous callus palpated with minimal instability. The right metacarpal region was lasered at 504 J for 5 minutes and 36 seconds. Then the splint was replaced and the wing was wrapped in a figure 8 pattern.

wing was aseptically prepared with dilute betadine. The sharp
bone edge was pale grey and of questionable viability. There
was also concern for being able to close the site over this protruding
piece of bone. Therefore, the sharp protruding portion of bone
was debrided with rongeurs and briefly filed with a rasp to smooth
the edges. The surrounding skin was freed/elevated. The wound
was apposed with 5-0 PDS, using catheter tubing to alleviate
skin tension on one of the sutures. Omnicide wound gel was applied
and the area was covered with a tegaderm+pad, SAM splint, and

------------- LASER THERAPY ---------------- Power (joules): 504 Time: 5 min 36 sec Location: right metacarpal region PROM performed under anesthesia. 017 was anesthetized with ISO (5%) at 3:11 PM. The elbow was at a normal ROM, while the carpus was at 160 degrees. After brief PROM, the carpus angle improved back to a normal 180 degrees. The right metacarpal region palpated as a firm firbrocallus with minimal to no instability. Fracture site was lasered (504 J) for 5 minutes and then secured with cast padding and vet wrap in a figure of eight pattern. We elected to remove the vetlyte splint in order to prevent any potential pressure necrosis of the fibrocallue's contact with the splint and the figure of eight was elected to prevent 017 from reinjuring it in the cage. HR & RR were monitored with auscultation during the procedure. Under anesthesia for 12 minutes. Patient recovery uneventful.

Examined by Dr. Czepiel Left eye (OS) entry: Normal PLR + Palpebral + Dazzle + Fluorescein - IOP 22 No other significant findings.
Right eye (OD) entry: Early phthisis bulbi, uveitis, suspect hypermature cataract, blind PLR - Palpebral + Dazzle - Fluorescein - IOP cornea too soft to measure (hypotony) Uveitis, early phthisis bulbi (smaller eye), suspect a hypermature lens, blind OD. Deep anterior chamber suspected from the changes in pressure of the eye. Mydriatic, 3-4+ flare, resolving retinal hemorrhage.
Recommendations: Continue meloxicam and ketorolac OD BID. Consider evisceration OD or increasing ketorolac to TID with a recheck in 2 weeks.

Images taken: Body VD, Right Lateral Wing, Right Extremity Cranial-Caudal Wing Radiology Interpretation: Fracture of both metacarpals. Major metacarpal has a closed, comminuted fracture of the diaphysis. Minor metacarpal has a closed, complete, short oblique diaphyseal fracture. No significant osseous callus formation. The fracture is chronic in nature and the fracture margins are ill defined. There are no obvious signs of healing if chronic in nature. Recommended recheck in 3 weeks to monitor healing progression. Anesthesia Protocol: 017 was anesthetized with isoflurane and was monitored every 2-3 minutes by checking heart rate and respiratory rate. The bird was maintained for a majority of the imaging procedure at 5 for gas anesthesthetic and switched to 4 closer to the end of our procedure. A figure of eight wrap was placed and gas anesthetic was ended. 017 was held in an upright position during recovery until they were able to hold their own head up. Then they were placed in a transport box to be moved back to SERC.

------------- PHYSICAL EXAM FINDINGS ---------------- Attending Veterinarian: Dr. Seth Oster Observational examination Mentation: Bright, alert, and responsive Posture: Upright Vital stats Dehydration: Euhydration Beak: Normal Cere/nares: Normal Eyes: The right eye was non-visual. The left eye had pigment clumping and a good PLR. Ears: Normal Wings: Right wing had a palpable metcarpal fracture that was in the beginning stages of healing. BCS: 3 Problem list: 1. Nonvisual right eye 2. Pigment clumping left eye 3. Fractured right metacarpal Diagnosis: 1. Nonvisual right eye 2. Pigment clumping left eye 3. Fractured right metacarpal Assessment: 2024-017 has a fractured right metacarpal bone that has begun to heal based on palpation. The wing was placed in a wing wrap for stabalization. The eyes were also examined and the right eye was completely non-visual. The left eye still had a good PLR but did have pigment clumping present. Further evauluation is need to assess the patient's visual abilities in the left eye. Plan: 1.Collect HPAI test tomorrow (3/21/2024) and send it to the state lab 2. Rdiograph right wing to see how far along the fracture has healed 3. Schedule ophthamology appointment to assess vision in the left eye. 4. Administer injectable meloxicam 0.13 ml for pain/inflammation 5. Administer Ketoralac to help with inflammation of the eye Prognosis: Fair
